On shared hosting do we have access to a temp folder above the public_html folder? Or can we create one? The reason I ask is when dealing with user uploads, it would be simple to upload say an avatar to a folder users cannot access, thanks for your time.

You do have folders above public_html, yes. But remember that img tags on your site won't have access to it-- just you via FTP and your php scripts.

Make sure that you set the right permissions for the folder and use a php script to read and echo the binary images.
