Database Help?

I've created a simple file based database using phpFormGenerator. I can input data without a hitch. Now I need to search the data and I can't get my brain wrapped around creating a Search form.

It's 7 fields of data. So I just need to tie in a search page that will contain an 8 choice dropdown (7 fields + ALL) and then a keyword text box. And then obviously a results/no match output page.

If someone can finish what I've started, or at least steer me in the right direction that would be great. I've input very little data, so if starting from scratch is a better solution I'm open to that.

Need only plain pages - I can customize the visuals.
