Default can't use the results by php FormGenerator

I installed phpFormGenerator, and set a test-database-table in MySQL, want to create a form in my website to transfer visitors information to the database in MySQL.
The form was created by phpFormGenerator, but as I tried to submit my-test-input, it failed! The prompt is
"Internal Server Error. The server encounted an internal error or misconfiguration and was unable to complete your request. ......"
Did anyone encounter this trouble? How to solve?

you'll need to look at your php error log to see what the exact error was before anyone else can help diagnose it.

I checked the error log, it is
[Mon Jun 06 19:26:21 2011] [error] [client 189.200.148.102] File does not exist: /home/zoey/public_html/favicon.ico

That's the web service error log. You need to look at the one for php. It should be in the directory with the script, or in your main directory, called error_log. Take a look at what's in it and let us know what's there.

What is the url of your form page?

There really is an error_log.php file, but I can't open it.

if your useing the phpform generator that is in fantastico you might have a problem. Unfortunetly as cool and easy fantastico is it does not install php databased scripts 100%. The best corse of action is to install it yourself.

If you did install it yourself the problem maybe permissions depending on what your trying to open it with. Also you can direct download the file, edit it, and reupload it.
Internal server errors are folder/file permissions usually. try changing the destination folder's permissions to 755 and see if that works.
