I've personally used clamav for real time e-mail scanning on some servers and it worked great. I know that it increases the load on the servers a little, however it also reduces the bandwidth usage by preventing download of infected messages.
Also I beleive it is a good marketting tool as well. I've seen other hosting companies advertising virus free mail. You may want to offer this service as well.

The problem is they are very very CPU intensive, we offer this on dedicated servers if the customer would like but currently have no plans for reseller or shared servers. We recommend users use a home solution to combat the problem and we already have filters in place that don't scan but do prevent .scr and other virus used attachments to be blocked.
